During loading operations of a CAT excavator at Manzanillo, Operator stopped at the break point of the ramp, sudden weight shift caused the tracks to lose grip. Unit slid laterally despite rubber mats. Excavator lost traction during the incline transition on the ramp causing structural damage to vessel bulkhead and potential injury risk to operator or nearby spotters
Lesson Learned
Ensuring safety culture behavior at ramp transitions helps prevent loss of control, and protects all personnel involved and vessel structures
